operate a home-like, family-centered, friendly facility committed
to caring! With only eighty residents on three levels, we focus
on each individual, providing the personal care needed to maintain
and improve the quality of life, while ensuring a safe environment.

too, is a prime concern at Hollis Park Manor Nursing Home.
With our residents’ well-being of paramount concern, we
offer a comprehensive program of both short and
long term care to meet individual needs. Our intimate setting
allows our staff the ability to become very familiar with each
resident and their family. Most of the employees at Hollis Park
have been with us for over ten years, enabling them to create
a comfortable, secure and nurturing environment. From the moment
of transition to our facility — and throughout the days
of residency — residents and family members feel welcome.

Care Plan: Every resident is evaluated
by the health care team to design an individualized care plan
specific to his/her needs.

residents, increase and improve physical and mental functioning.

Dental and Ophthalmology Care

services are provided to all residents.

a registered dietician addresses all special dietary needs,
including individual requests for the three
meals and two daily snacks.

residents needing dialysis are transported to nearby facilities.

“our family” to yours… Everyone at
Hollis Park is dedicated to upholding the highest standards of
care. We are a facility that works as a team to ensure the best
possible experience. Staff, residents and family members have
open access to expressing their concerns and needs. The Administrator,
Director of Nursing, Social Worker, Dietician and Recreation Director
meet regularly to assess day-to-day needs and concerns. IV certified
licensed professional nurses are on site 24 hours a day. Post-surgical
care and specialized skin treatments are a primary concern of
our nursing staff.

Hollis Park Manor Nursing Home is licensed by New York State and
operates in compliance with all local, state and federal regulations
which prohibit discrimination based on race, religion, creed,
color, national origin, sex, age, sexual preference, sponsorship
or source of payment. Medicaid, Medicare and private payments
cover the cost of daily care.
Member of the New York State Health Facilities Association and
Greater New York Health Care Association.
All
requests for admission are carefully reviewed in a timely manner.
New residents can be admitted 24 hours a day, every day of the
week. Assistance with Medicaid applications is provided.
